One (1) winner will be selected in a random drawing from all properly completed entry forms. Entries may be submitted by 1) completing and mailing a printed business reply card, 2) completing and submitting a printed entry card at the Instrumentarium Dental tradeshow booth, or 3) completing an on-line form at www.instrumentariumdental.com The drawing will be held on or about December 1, 2015 by the sponsor Dental Imaging Technologies Corp. d/b/a. Instrumentarium Dental, whose decision is final. Participant must be present to win. Sweepstakes open to practicing US-resident dentists only. One entry per person, and the sweepstake will not be combined with any other Instrumentarium Dental promotions. Odds of winning are determined by the number of entries received. Instrumentarium Dental is not responsible for lost, late, illegible, unintelligible, incomplete, undelivered, stolen, postage due or misdirected entries. Prize is one (1) OP300 Maxio $25,000 coupon ($25,000 prize value toward purchase of OP300 Maxio). Sponsor shall only be responsible for awarding the number of prizes as stated in these Terms and Conditions. Prize is non-transferable, cannot be substituted and has no cash alternative. By entering this Sweepstakes, participant acknowledges that the information provided may be used to communicate with the participant in relation to this Sweepstakes as well as for other purposes. Participants DO NOT have to submit to a sales presentation.
